AVENTURA GOVERNMENT CENTER 2999 NE 1915T STREET SUITE 500 AVENTURA, FLORIDA I. Call to Order. The meeting was called to order by Chairperson Janet Laine. City Clerk Teresa M. Smith called the roll. Members present: Elaine Baxter Irving Kanowitz Janet Laine Lenore Toby Simmons Members absent: Mindy Bloom Ruth Demming Don Gustin Others present: Community Services Director Robert M. Sherman Commissioner Harry Holzberg A motion to excuse the absences of Ms. Bloom, Ms. Demming and Mr. Gustin was offered by Mr. Kanowitz, seconded by Ms. Baxter and unanimously passed. II. Approval of Minutes. A motion for approval of the minutes of the January 25, 1999 meeting was offered by Mr. Kanowitz, seconded by Ms Simmons, and unanimously passed. III. Chairman's Report. Ms. Laine requested that Mr. Sherman provide to Board members at the next meeting a recap of all programs and events recommended by the Board to date. ? ~~ Board members discussed possible locations for holding events. Mr. Sherman and Ms. Simmons to determine the feasibility of holding events at the AMC Theater and the Point East Auditorium. IV. Staff Report. A. Concert Updates. Mr. Sherman provided an update on the negotiations with FlU for a concern in the park on April 18th. He also discussed the upcoming July 4th fireworks display. B. Aventura SonQ. Mr. Sherman discussed the contest requirements. A motion to proceed with recommendation of contest was offered by Mr. Kanowitz, seconded by Ms. Simmons and unanimously passed. V. MeetinQ Dates: Ms. Laine announced the next meeting will be held on March 15,1999 at 9:30 am. VI. Other Business: None. VII. Adioumment. There being no further business to come before the Board, the meeting adjourned at 2:05 p.m. Teresa M. Volunteers must be 16 years or older and have their own transportation. . Volunteers are to be groomed and in clean attire. . The City of Aventura is not responsible for your valuables. (Please refrain from bringing any valuables.) Schedule: . Volunteers will receive a posted schedule. If a volunteer can not perform their assigned shift, call the Community Services Department at 305-466-8930. Try to allow at least two days notification. . If after hours, leave a message on the voice mail. . Volunteers are to report to their assigned shift ten minutes before scheduled time. It is the responsibility of the volunteer to be on time. Volunteer Guidelines/Under Supervision: . Volunteers will assist in the following areas: greeting at special events, seating at special events, ticket taking at special events, assisting with cultural field trips, assisting with park field trips, park operations and programs, office assistance, mailings, preparation of reports and other related tasks. . Each volunteer must be able to stay in their assigned area throughout their shift. . Each volunteer will work in a specific area, group or activity and provide friendly courteous assistance at all times. . A volunteer will offer the best assistance, promote the department, secure the safety of themselves and those around them and improve communication with neighborhood and community groups in respect to their usage of Community Service Programs. 1 TraininQ: . All volunteers will be oriented with the objectives and philosophy of the Community Service Department. . Each volunteer will sign for a copy of nrles and policies to be observed. . Each volunteer will be educated in his or her area of interest, and cross- trained to assist in areas as needed. . Each volunteer will receive a copy of safety procedures to be followed in the event of an emergency or accident. Supervision: . Each volunteer will report to the supervisor in charge and receive their job assignment. (Supervisor may vary) Compliance to Hours: . All volunteers must log in and out per each function/event and be limited to a twenty hour per week. A volunteer week runs from Thursday to Wednesday. Volunteers need to possess: . A desire to be in community service and contribute to the quality of life. . A personal need for a sense of achievement and participation in a group situation. . A desire to continue to be involved and active in a special interest group that is self-gratifying and enriching to your life. . A willingness to increase your knowledge of Community Services and assist patrons with special needs. RecoQnition of Volunteers: . A volunteer is a major contributor to the City, and will be recognized each year. Attirelldentification: . Volunteers will be required to wear a volunteer badge and uniform at all times. . Volunteers are to wear comfortable closed- toed shoes. Evaluations: . A volunteer will be evaluated annually on their job performance and expectations. . Evaluations will be based on: performance of tasks, attendance, attire, inter-personal communication skills. . Volunteers will be given every opportunity to achieve their ambitions, however, in rare instances, if deemed necessary; a volunteer may be dismissed. 2 ~ ' / q, .J. ../..A\) ~ G~-~jE~CeW" HOLD HARMLESS AGREEMENT - VOLUNTEERS FOR AND IN CONSIDERATION OF THE CITY OF AVENTURA, permitting me to volunteer my time, I hereby agree to waive and hold harmless the City of Aventura, its officers, agents, and/or employees, heirs, executors, administrators, successors, assigns employees, agents, representatives and all other persons, partnerships, firms or corporations of and from any and all actions, causes of action, damages or demands of whatever name or nature in any manner arisen, arising or to grow out of any and all matters against any and all claims that may incur, including but not limited to an injury or accident while donating my time, and by executing this document, I hereby agree to assume the risk of any incident that may occur while I am volunteering my time. It is further acknowledged that there is no agreement or compromise on the part of said City of Aventura, its officers, agents, and/or employees to do or omit to do any act or thing not herein mentioned, and that the below signed declares that they are over 16 years of age, and that I rely wholly upon my own judgment, belief and knowledge of the nature, extent and duration of my desire to be a volunteer for the City of Aventura. I herein release the City of Aventura, its officers, agents and/or employees and further state that they have not influenced me in making, or induced me to sign this agreement. Further I do covenant to indemnify, hold harmless and defend the City, its agents, from any claim, liability, damages or demand hereafter arising out of any injury to said child, regardless of whether such injury is caused by the negligence of said City or its agents, servants and employees. Participation in the City of Aventura Theme Song Contest is available to local businesses, agencies, public schools, organizations and clubs. Eligibility: · Applicant(s) must be 18 years or older and an Aventura Resident. Participants under 18 must submit a parental release form signed and notarized by a parent or legal guardian. · Applicant(s) must submit entry form with a brief bio of authors. Rules: . Song should be no longer than 3 minutes. . No foul language permitted in content of song. . Theme song must be original work. · Song may not discriminate on the basis of race, disability, ethnicity, sex, creed or religion. . Song may be edited for style, length or clarity. Song/Lyrics presentation: · Typed on 8 % x 11 white paper; double-spaced and VHS videotape and audio tape must accompany lyrics. . Song will be published and distributed by the City of Aventura. . Unveiling of Song/Lyric: · Song will be sung at the Annual July 4th Celebration, Sunday, July 4, 1999. Judging Categories: . Special recognition will be given to winners. . All entries will receive a certificate of recognition. . The Recreation Cultural Board will review all applicants. · The City of Aventura and City Manager reserve the right to defer the review of an applicant. · The City Manager will present recommendations to the Mayor and Commissioners for final adoption. .
