Resolution No. 2021-44 Awarding Group Health Insurance Program to UnitedHealthCare and Employee Assistance Program to Aetna - July 22, 2021 CITY OF AVENTURA RESOLUTION NO. 2021-44 A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF AVENTURA, FLORIDA, AWARDING A CONTRACT FOR THE CITY'S GROUP HEALTH INSURANCE PROGRAM TO UNITEDHEALTHCARE FOR ITS CHOICE PLUS BWRJ WITH RX 124X AND V1458 FOR HEALTH AND VISION INSURANCE AND SELECT AETNA FOR THE EMPLOYEE ASSISTANCE PROGRAM (EAP) AS SPECIFIED IN THE REQUEST FOR PROPOSALS ("RFP"); AUTHORIZING THE CITY MANAGER, ON BEHALF OF SAID CITY, TO EXECUTE ASSOCIATED CONTRACTS; AUTHORIZING THE CITY MANAGER TO DO ALL THINGS NECESSARY TO CARRY OUT THE AIMS OF THIS RESOLUTION; AND PROVIDING FOR AN EFFECTIVE DATE. WHEREAS, the City of Aventura has employees rendering valuable services; and WHEREAS, the City desires to maintain a Group Health Insurance Program and Employee Assistance Program (EAP) which offers good benefits at an affordable cost to the City and its employees; and WHEREAS, the City has properly solicited and received requests for proposals for health, vision and Employee Assistance Program (EAP) insurance coverage for City employees; and WHEREAS, the City's Group Insurance Broker/Consultant has reviewed the various proposals and, along with City staff, recommends awards of contract to UnitedHealthcare for its Choice Plus BWRJ with RX 124X Plan and V1458 for health and vision insurance and selects Aetna for the Employee Assistance Program (EAP) as specified in the RFP; and WHEREAS, the City Commission, upon the recommendation of City staff, desires to award contract to United Healthcare for health and vision insurance and Aetna for the Employee Assistance Program (EAP). NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF AVENTURA, FLORIDA, THAT: City of Aventura Resolution No. 2021-44 Section 1. The contract for employee health and vision insurance be awarded to UnitedHealthcare and the Employee Assistance Program (EAP) be awarded to Aetna in accordance with the RFP submitted by said company. Section 2. The City Manager is hereby authorized to execute, on behalf of the City, a contract by and between the parties embodying the terms, conditions, and specifications as set forth in the subject invitation, specifications, proposal and requirements for employee health and vision insurance, as well as the Employee Assistance Program (EAP). Section 3. The City Manager is hereby authorized and requested to take all necessary and expedient action to carry out the aims of this Resolution. Section 4. This Resolution shall become effective immediately upon its adoption. The foregoing resolution was offered by Commissioner Evans, who moved its adoption.
